OceanGate had critically flawed safety practices

WASHINGTON: The US Coast Guard Marine Board of Investigation released a 300-page report Tuesday (5th Aug 2025) following a 2-year probe on the implosion of submersible Titan (that was on a tourist expedition to Titanic wreckage) in 2023.

Titan harboured five voyagers including 2 Pakistanis: 48-year-old businessman Shahzada Dawood and his 19-year-old son Suleman Dawood.

According to the report Titan’s inadequate design played a key role in its implosion. Its remains were found four days later, littering the seabed about 488 metres from the bow of the Titanic wreck.

“For several years preceding the incident, OceanGate leveraged intimidation tactics, allowances for scientific operations and the company’s favourable reputation to evade regulatory scrutiny,” the report adds.

OceanGate (that was suffering from lack of funds), qualified pilots (sans prior experience of operating a submersible) even in one day.

According to Jason Neubauer, the head of the probe, the accident was avoidable: “There is a need for stronger oversight and clear options for operators who are exploring new concepts outside of the existing regulatory framework.”
